HUFC ANNUAL MEMBERSHIP APPLICATION

I wish to apply to become a member of Hadleigh United Football Club for the 2009//2010 season.
I agree to abide by the rules of the Club.

Please complete the following form and return it with the appropriate membership fee to either Chris Rose or your Sons Youth Team manager.

Notes:-

1. Family membership is for up to two parents / guardians and three children under 18. Please list the names of all family members to be covered by the membership.
2. Youth Player Family membership is for up to two parents / guardians and three children under 18. At least one of the children must be registered HUFC youth players. Please list the names of all family members to be covered by the membership.
3. Appropriate Membership Fees must be paid at the time of the application. Please provide cash or a cheque made payable to Hadleigh United Football Club.
4. Membership card(s) will be issued to indicate acceptance of membership application.
5. Club Rules are available for reading at the Clubhouse. Copies can be provided on request.
